Clinical Pharmacist around Woodford, London
to work in Primary Care
We are looking for a Clinical Pharmacist to join a PCN in Woodford, London. This is an opportunity to make a difference everyday and deliver exceptional service to the community. The post holder will be responsible for their clinical competencies as part of a multi-disciplinary team and provide expertise in clinical medicines management. The post holder will also provide face-to-face structured medication reviews, manage long-term conditions, management of medicines on the transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, and acute prescription request while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN. There will be support for career progression.
Must be registered as a Pharmacist with the GPhC
📍Location: Woodford, London
🗒Contract: Fixed term
💷Salary: £45,000- £50,000
⏰Hours: Full time
✅Benefits: 25 days annual leave plus bank holidays
